Joint & Musculoskeletal Injections
In-office evaluation and treatment for knee, shoulder, and hip bursa pain — including ultrasound-guided corticosteroid injections — serving Northville, Plymouth, Canton, and Livonia.
Joint pain is one of the most common reasons adults see a doctor, and one of the most common reasons they're sent somewhere else by default. Many cases of knee, shoulder, and hip bursa pain don't require a trip to an orthopedic surgeon at all. At TheMDClinic in Northville, MI, we evaluate and treat these conditions directly, including ultrasound-guided corticosteroid injections, so a swollen knee or a painful shoulder doesn't have to mean a multi-week wait for a specialist appointment.
What We Treat
Knee Osteoarthritis
The single most common reason for joint injections in general practice, providing relief from chronic wear and tear.
Shoulder Conditions
Including subacromial bursitis, rotator cuff tendinopathy, adhesive capsulitis ("frozen shoulder"), and impingement syndrome.
Trochanteric Bursitis (Hip Bursitis)
Treating lateral hip pain that is frequently mistaken for an internal hip joint problem.
Gout & Inflammatory Flares
Joint aspiration to confirm the diagnosis, and targeted injection for fast, effective symptom relief.
Acute Joint Effusions
Aspiration to relieve painful pressure and send fluid for lab analysis when infection or crystal disease needs to be ruled out.
Ultrasound-Guided Injections
We use point-of-care ultrasound to guide our joint and bursa injections, which meaningfully improves needle placement accuracy compared to a landmark-based approach alone, particularly for patients with a higher BMI, prior joint surgery, or a smaller effusion that's harder to localize by feel. Ultrasound guidance also means less guesswork and less discomfort during the procedure itself.
How We Approach Dosing
We use the lowest effective corticosteroid dose for each joint, based on current national guidelines, rather than defaulting to the highest dose available. For most shoulder injections, a lower dose has been shown to work just as well as a higher one; for the knee, a moderate dose performs as well as a much larger one. We also space repeat injections out appropriately, since injecting the same joint too frequently can have a cumulative effect on joint cartilage over time. Less medication, used thoughtfully, is often the better choice — not the lesser one.
What to Expect, and Who Should Be Cautious
Most patients tolerate joint injections well, with relief that typically begins within a day or two and can last anywhere from a few weeks to a few months. If you have diabetes, it's worth knowing that a corticosteroid injection can cause a temporary rise in blood sugar lasting anywhere from several hours to up to two weeks, so we'll talk through how to monitor for that. If you're on blood thinners, we'll review your specific medication with you — most patients can safely receive joint injections without stopping anticoagulation, since the data shows the bleeding risk from the injection itself is very low. Injections aren't appropriate for everyone — active infection at or near the injection site or a suspected fracture may change the plan, and we'll always review your history first.
Diagnosis First, Not Just Treatment
A swollen, painful joint sometimes needs answers before it needs treatment. Joint aspiration lets us send fluid for analysis to distinguish gout from other types of arthritis, or to rule out joint infection, which is a different and more urgent problem entirely. We perform this in-office rather than routing you to an emergency room for a question that can often be answered right here.
When We Refer to Orthopedics
We're upfront about where the line is. If an injection doesn't meaningfully help after one or two attempts, if imaging suggests a structural issue like a significant meniscal or rotator cuff tear, or if surgery is genuinely the right next step, we refer you to an orthopedic specialist, with clear clinical context already documented so your first visit there is productive rather than a repeat of everything we've already done.
